Add catalog-backed book cover extraction support
Implement cover extraction for books stored in catalog.fibc omnibus format to avoid loading large (~400KB) book arenas into RAM. Uses catalog's pre-resolved cover entries with minimal memory footprint (~42KB resident). Includes arena management for catalog opening and ZIP entry extraction with configurable scratch buffers.
